A home care agency in Raleigh is looking for a Referral Liaison to promote the Agency's services and develop referral relationships within the community.

The Referral Liaison shall:

Establish and maintain relationships with hospitals, physician offices, rehabilitation facilities, case managers, social workers, senior centers, and other referral sources.
Educate referral sources regarding Agency services.
Conduct marketing visits and community outreach activities.
Track referral activity and maintain records of contacts and outcomes.
Submit weekly and monthly activity reports.
Represent the Agency professionally and ethically at all times.

Compensation is negotiable.
